چکیده

A 64-year-old man with T2N0 squamous cell carcinoma of the tongue base underwent treatment with radiation, followed by glossolaryngectomy, bilateral neck dissection, and placement of an anterolateral thigh flap graft. After 1 year, he presented with acute cholangitis (abdominal pain, jaundice, fever, chills, and hypotension), making emergency endoscopic retrograde cholangiopancreatography (ERCP) mandatory. A computed tomographic scan of the neck showed hypopharyngeal stenosis and neck swelling as a result of the previous surgery (●" Fig.1a, ●" Video1). At another institution, emergency ERCP had failed, and the patient was transferred to our institution for further treatment. On standard esophagogastroduodenoscopy, the changes due to the prior com-

Insertion of fully covered self-expandable metal stent during balloon-assisted ERCP in patient with surgically altered upper gastrointestinal anatomy.

The presence of complex, surgically altered gastrointestinal (GI) anatomy poses a diagnostic and therapeutic challenge to the endoscopist treating patients with pancreaticobiliary disorders. Placing fully covered metal stents into the bile duct during endoscopic retrograde cholangiopancreatography (ERCP) through standard forward-viewing scopes, such as doubleand single-balloon enteroscopes and ...

Overtube-assisted placement of a metal stent into the bile duct of a patient with surgically altered upper-gastrointestinal anatomy during double-balloon enteroscopy-assisted ERCP.

Wehere report on a novel approach to deliver a fully covered self-expanding metal stent (SEMS) into the bile duct during double-balloon enteroscopy (DBE)-assisted endoscopic retrograde cholangiopancreatography (ERCP) using the overtube as an “accessory” or working channel. An 87-year-old woman with a history of a Billroth II reconstruction many years previously and complex choledocholithiasis h...

Utility of the balloon-overtube-assisted modified over-the-wire stenting technique to treat post-sleeve gastrectomy complications

AIM To describe a modified technique of deployment of stents using the overtube developed for balloon-assisted enteroscopy in post-sleeve gastrectomy (SG) complications. METHODS Between January 2010 and December 2015, all patients submitted to an endoscopic stenting procedure to treat a post-SG stenosis or leakage were retrospectively collected. Procedures from patients in which the stent was...

who presented with obstructive jaundice due to a large (2.5 cm) stone in the common bile duct (CBD). We attempted mechanical lithotripsy but the stone was too big for the basket (●" Fig. 1). We decided to break the stone with electrohydraulic lithotripsy (EHL) and carry out direct peroral cholangioscopy instead of motherbaby endoscopy. In our experience, the mother-baby endoscopic system has se...
